Diabetes and cardiovascular disease (CVD) are significant global health challenges. Pharmacists play a key role in early detection, management, and prevention, reducing the burden of these conditions on healthcare systems. With rapid point-of-care testing, pharmacists can screen patients, identify risks early, and provide timely interventions.

This educational module is designed to enhance pharmacists’ ability to address diabetes and CVD risks in adults. It provides training on risk factors, screening, management, risk assessment, intervention, and lifestyle modification strategies. By applying this knowledge, pharmacists can effectively identify at-risk patients, optimise chronic disease management, and improve health outcomes for those with diabetes and CVD.


Learning outcomes: 

  1. Describe the Role of Pharmacists in Chronic Disease Management – Learn how pharmacists can play a key role in screening, managing, and preventing diabetes and cardiovascular disease through point-of-care testing.
  2. Identify High-Risk Patients and Provide Targeted Interventions – Develop skills to assess patient risk factors for diabetes and cardiovascular disease and implement evidence-based interventions.
  3. Implement Patient Counselling and Lifestyle Modification Support – Learn how to provide effective counselling on lifestyle changes, medication adherence, and risk reduction strategies.
